INSTRUCTIONS TO UPLOAD HEALTH REQUIREMENTSAll incoming Freshman are required to submit:
STUDENT EMERGENCY INFORMATION - Due by March 29, 2024
CERTIFICATE OF CHILD HEALTH EXAM - Due by May 1, 2024
Health examination with proof of immunizations prior to the first day of school. This health examination must be completed by a MD, DO, APN or PA on or after 8/14/23. The Health History section on page 2 must be completed and signed by the parent/guardian.DENTAL EXAMINATION - Due by May 1, 2024
Dental examination must be completed by a licensed dentist on or after 11/15/23.
